Student-athletes entering grades 7-12 for the 2026-27 school year are invited to complete their required pre-participation sports physicals ahead of the upcoming athletic season.

Each student must have the Ohio High School Athletic Association Pre-Participation Evaluation (PPE) form for the 2026-27 school year completed by a physician and signed by a parent or guardian prior to participation

Students attending the clinic are encouraged to wear athletic attire and bring contacts or glasses if needed during the evaluation.
